# Putting the waiver online — setup guide

You pick the service and create the account (I can't sign up or enter payment details).
Everything below is written so it's paste-and-go once you're in.

## Which service

| | WaiverForever | Smartwaiver |
|---|---|---|
| Free tier | **Yes** — Starter, limited | No |
| Entry paid plan | $19.99/mo Essentials | $19/mo Basic (100 waivers/mo) |
| Next tier | $39.99/mo Standard | $55/mo Starter (300/mo) |
| Dormant-month option | — | **$7/mo storage plan** |
| Kiosk mode (tablet at the door) | Yes, iOS + Android | Yes |

**Start on WaiverForever's free Starter plan.** You will have very few bookings in month one,
and there is no reason to pay before there are events. Upgrade when volume justifies it.

**The one reason to prefer Smartwaiver:** its **$7/month storage plan** for quiet months. A
weekend venue has dead stretches, and $7 to keep the archive live beats $19–20 for a month with
two bookings. If you expect long gaps, Smartwaiver + storage plan is the cheaper year.

Either is fine. Both give you the thing that matters: a signature with a timestamp, an emailed
copy to the signer, and a searchable archive.

## Before you build the form

**Get page 2 checked by a Florida attorney first.** The minor-release notice has to match
Fla. Stat. § 744.301(3). Putting an unenforceable clause online just collects unenforceable
waivers faster. Everything else can go up now.

## Fields to create

### Participant details (the platform gives you these)
- First name, last name — **required**
- Email — **required** (this is how they get their copy)
- Phone — required
- Date of birth — optional
- Address — off, you don't need it

### Custom questions to add
| Question | Type | Required |
|---|---|---|
| Date of your event | Date | Yes |
| Which space(s) are you booking? | Text | Yes |
| What are you celebrating? | Text | No |
| Approximate number of guests | Number | Yes |
| Emergency contact — name | Text | Yes |
| Emergency contact — mobile | Phone | Yes |
| Names and ages of any children in your care | Long text | No |

### Checkboxes — one per clause, each required
Do **not** collapse these into a single "I agree". Separate ticks are what shows each term was
put in front of them.

- [ ] I am 18 or older and I am the person responsible for this booking.
- [ ] I understand children must be supervised by an adult at all times, and that Shelton staff do not supervise children.
- [ ] I have read the campus rules: no alcohol without prior approval, no smoking or vaping, no open flame, nothing fixed to any surface, no confetti or glitter.
- [ ] I accept responsibility for damage beyond normal use, and for anything left behind.
- [ ] I will make sure any vendor I bring provides a certificate of insurance before the date, and I understand no certificate means no access.
- [ ] I have read and accept the assumption of risk and release of liability above.
- [ ] I agree to the payment terms: 30% plus a $200 refundable damage deposit to hold the date, balance due two weeks before, Zelle.

### Photo and video — a required choice, not a tick
Make this a **required multiple-choice with two options**, so nobody skips it and a blank is
never read as consent:

> **May we photograph or film your event for our marketing?**
> - Yes — Shelton After School may photograph or film my event and use the images on social media, its website, print and email.
> - No — please do not photograph or film my event for marketing.

Add below it, as help text:
> Either answer is fine and it changes nothing about your booking. Any guest may ask a staff
> member not to be photographed on the day. To have something removed later, write to us and we
> will take it down from anything we control.

### Signature
- Drawn signature — **on**
- Typed name confirmation — **on**
- Auto-email a copy to the signer — **on** (this is the setting that matters most)

## The waiver body text

Paste `waiver-body-for-platform.txt` into the main agreement box. It is the same wording as the
printed PDF, with the form fields stripped out.

---

## After it is live

1. Put the waiver link in the **booking app** — there is a field for it on every booking, and a
button to send it to the client on WhatsApp.
2. Add it to the site's **Questions** section.
3. Send it with the deposit request, not on the day. People sign what they are asked to sign
before they have paid attention to anything else.
4. **Keep the printed PDF too.** Grandparents, spotty signal, a phone that dies. A paper copy at
   the door costs nothing.
5. Export the archive **once a quarter** and keep it somewhere that is not just the platform.

## Retention
Keep signed waivers for **at least 4 years** after the event — Florida's negligence limitation
period is generally 4 years for incidents from 2023 onward, and longer for a minor, whose clock
can start at 18. Your attorney should confirm the number for your situation; the safe habit is
simply never to delete them.
